POSITION SUMMARY:
The Palliative Care Coordinator (PCC) is an experienced professional who initiates and coordinates the Palliative Care plan and coordinates the services provided to Palliative Care patients. The PCC participates in patient care conferences and works cooperatively with the attending physician, Palliative Care Medical Director, and other members of the Palliative Care team. The PCC provides coordination of activities of assigned RNs, LVNs and NPs. The PCC provides coordination of services to patients who have been admitted for Palliative Care services. The PCC regularly coordinates referrals to the Palliative Care program, assures proper authorization for care, assures visits are scheduled as authorized and that documentation by care team is conveyed to PCPs and authorizing managed care groups.
- Graduate of an accredited school of vocational nursing.
- Current LVN license within the State of California. Certification in Hospice and Palliative Nursing encouraged.
- Experience in Palliative Care highly recommended.
- Spanish speaking a plus, but not required.
- At least one year of LVN experience.
