SalaryJob Duties:
-Verifying insurance and benefits of patients prior to the start of care, responsible ion explaining the hospice benefit to patient/ family to obtain proper documents for admission. Coordinate the initial care to doctors and the hospice team, ordering of DME, Supplies, Medication as needed.
-Ensuring home charts for active patient are complete and updated.
-Scheduling visits for all disciplines, which includes MD, RN, LVN, HA, MSW, SC
-Must be willing to do PRN visit during office hours (if needed)
Qualifications:
-Active license as a Vocational Nurse (LVN)
-Experience working with hospice patients is required
-Ability to work independently with minimal supervision
Job Type: Full-time
Pay: $30.00 - $35.00 per hour
Expected hours: 40 per week
