Are you a Licensed Practical Nurse (LPN) seeking an opportunity to care for others through HomeCare? MCS is seeking a dynamic and energetic LPN for in home member services. Our office is located right in downtown Springfield, MA! Metrocare has been delivering top notch Home Health Care services for over 10 years!
Responsibilities and Skills of the LPN/RN
- Complete all applicable clinical assessments.
- Develop and review members' plan of care.
- Monitor health status of all members and document accordingly.
- Conduct on-site Licensed Practical Nurse (LPN) visits.
Requirements andQualifications
- Licensed Practical Nurse (LPN) must be fully licensed in the Commonwealth of Massachusetts.
- Ability to establish and maintain professional relationships with members, care-givers, co-workers and community leaders.
- Ability to independently organize work and meet deadlines.
- Bi-Lingual Spanish/English Preferred
