We are a team of Qualified Mental Health Professionals that include, Licensed Clinical Social Workers, Licensed Clinical Professional Counselors, Licensed Marriage and Family Therapists, Nurse Practitioners, and Holistic Wellness doctors.
We are hiring a part-time Intake Coordinator to facilitate and coordinate the timely evaluation and acceptance of clients referred for services with varying presenting problems. Flexible scheduling between the hours of 8:00am -8:00p.m 7 days a week.
This is an Independent Contract position located in Chicago
Responsibilities include, but are not limited to:
Compete intake assessments an IM+CANS with clients
Assesses patients throughout their episode of care to determine medical necessity and create an individualized plan of care for the identified diagnosis.
Provides crisis/linkage case management to ensure continuity of care.
Complies with productivity standards.
Demonstrates sensitivity and responsiveness to cultural differences present with service populations.
Completes all required documentation in EHR system in a timely, ethical and thorough manner.
Provides telehealth services as needed in compliance with LSSI, insurance and government standards.
Maintains a small individual therapy caseload and other related tasks as needed.
Other assigned duties as needed.
Requirements include:
MA/MS in Social Work, Counseling, Psychology required or other Social Service filed required.
Licensed Clinical Social Worker or Licensed Clinical Professional Counselor preferred.
Ability to work a minimum of 15 hours per week.
