Position Summary
The Intake Coordinator for NYAP's La Jornada (The Journey) Program focusing on Transitional Foster Care (TFC) and Long-Term Foster Care (LTFC) for youth who have fled their country of origin and arrived in the U.S. with or without a parent or legal guardian, is responsible for overseeing the intake team responsible for recommending appropriate services and referrals coming into the agency.
RESPONSIBILITIES
- Perform all work in a manner consistent with the National Youth Advocate Program's mission, values, and philosophies.
- Responsible for the placement procedures, interagency correspondence and required agency case file information.
- Responsible for ensuring required and timely initial assessment of referrals.
- Responsible for recommending the appropriate service(s) to be delivered in accordance with the least restrictive environment, based on assessment results.
- Responsible for ensuring appropriate and timely communications to referring agencies and foster parents for the determination of the level of care of the clients, based on assessment results.
- Assist with increasing the referral base by approved methods and strategies to keep child-placing agencies aware of NYAP's continuum of care and services.
- Ensure data is gathered then compile, analyze and distribute statistical reports to supervisors detailing service reception activities.
- Enter all foster parent and youth information into the computer and update all changes, additions, deletions, etc.
- Advocate on behalf of the youth through individual and system advocacy to promote the provision of service delivery to the youth and their families via the least restrictive service delivery option available.
- Follow agency procedures for weekend and holiday emergency coverage and be available to potentially work a non-traditional work schedule.
- Occasional overnight travel may be needed.
- Interact effectively with internal and external stakeholders.
- Perform any other duties as assigned by leadership or designee.
MINIMUM QUALIFICATIONS
- A Bachelor's Degree in a social service or related field is preferred.
- Minimum two years' experience in youth services, preferring direct experience in child welfare.
- A valid social work license is preferred.
- Bilingual English/Spanish fluency is required.
- Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
- A willingness to work flexible and non-traditional hours with a possibility of being on call on nights/weekends, in the service of foster caregivers, families of origin, and persons served.
