QUALIFICATIONS FOR INTAKE COORDINATOR:
1. At least one experience in health care management, preferably in home
care operations.
2. Demonstrates good communications, negotiation, and public relations skills.
3. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
BRIEF JOB DESCRIPTION FOR CARE COORDINATOR:
1. Coordinates all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
2. Coordinates the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
3. Maintains comprehensive working knowledge of Advantage Health Systems contractual relationships and ensures that patients are admitted according to contract provisions.
4. Participates in quality assessment performance improvement teams and activities.
