Under the direction of the Director of Health Information, the HIM Clerk performs a variety of tasks associated with the processing and maintenance of patient medical records, working with the Providers to obtain complete medical records, updating/correction of transcription reports, processing of discharged charts for coders, outpatient coding, clinical documentation improvement reviews, concurrent and/or retrospective analysis of the medical record, locating and delivering records, destruction of records. Release of Protected Health Information (PHI) in accordance with State and Federal rules and regulations. Effectively facilitating chart completion by the Medical Staff. Demonstration of positive attitudes and customer service skills with patients, staff and physicians.
III. ESSENTIAL FUNCTIONS:
- Maintaining accurate incomplete/delinquent medical record statistics by physician.
- Assisting Physicians with completion of records.
- Timely processing of Transcribed reports which includes download of transcribed reports from a Web based transcription service; scanning and filing of reports in inpatient records.
- Release of Patient Medical Information in accordance with State and Federal Regulations.
- Clinical Documentation improvement Reviews as needed.
- Coding of outpatient medical records.
- Mantainence of case mix data for department reporting.
- Appropriate and cooperative Interface with all Departments
- File, copy and fax
- File and/or retrieve records safely from storage which involves picking up boxes which are approximately 35 pounds.
- Purge records for storage including safely moving boxes filled with records which are approximately 35 pounds.
- Maintenance of statistical data for reporting requirements
- Demonstrate the ability to work independently and prioritize projects
- Demonstration of positive attitude and customer service skills

IV. EDUCATION/EXPERIENCE REQUIREMENTS:
High school graduate or equivalent, typing speed 35 wpm, working knowledge of computers, Windows based programs preferred. Proficiency in alphabetical and numerical filing systems. Effectively able to communicate verbally and in writing, organizational skills. Ability to work effectively, uses good deductive reasoning, shifts priorities smoothly, and ability to function independently within established guidelines. Excellent time management and customer service skills. Excellent clerical and general office skills. A minimum of 1-year experience in office work, HIM experience preferred.
