Job Title: Designated Coordinator

Job Type: Salaried

Salary: $38,000-$47,600

Part Time/Full Time

POSITION SUMMARY:

The Designated Coordinator is responsible for providing supervision, support, and assisting with staff development for DSP staff. The Designated Coordinator complies with the Support Plans for individuals receiving services according to federal and state regulations and the organizations policies and procedures to ensure the quality delivery of services to all individuals served, as well as serving as a first responder to staff and residents emergency needs.

Essential Duties and Responsibilities:

Staff Supervision:

Assist with training, shift scheduling and ongoing support and evaluation of direct care staff.

Communicate challenges with staffing and/or service recipients issues to management.

Respond to crisis when needed/asked to do so.

Transport to/from school and or appointments

Assist with managing the DSPs task completion, training, documentation, reporting concerns to the necessary admin.

Enforcing policies and procedures and 245D compliance and paperwork.

Organizing and facilitating a monthly team meeting complete with meeting minutes.

Promoting a positive teamwork environment.

Assisting with the Implementation of new processes with/for staff.

Managing services, which may be hourly, or 24-hour support.

Problem solving when emergencies arise.

Other duties as assigned.

Client Supervision:

Evaluation of the effectiveness of service delivery, methodologies, and progress on the Individual served outcomes.

Communicating with individuals, guardians, case managers, behavior analysts, and other team members to decide on services needed to support everyone most effectively.

Working directly care as needed.

Site Responsibilities:

Ensuring the completion of service recipient programming, activities, etc.

Make recommendations to management regarding service recipients programming (introducing new, modifying, or eliminating activities

Communication:

Observe individuals for evidence of injury or bruises and evaluate for changes in emotional and physical status.

Report any noted problems to the proper persons immediately and complete a written incident report form.

Acts as a representative for Your Home Care Services LLC in a professional manner when required, to families, service coordinators, State officials, community partners, local businesses, and employers.

Maintains resident confidentiality.

Maintain confidentiality of the administrative team

Communicates with Management when items related to individuals personal needs and personal space needs to be purchased.

Treats all individuals and coworkers with dignity, respect, and fairness.

Provides support to other team members when individuals are having behavioral issues.

Documentation:

Documents all program data, including but not limited to individual plan data, behavior plan data, community integration logs, assessments, individual funds requests, mileage logs, maintenance requests forms, and supply acquisition forms.

Ensures that all documentation, staff and resident related is completed accurately and in a timely manner.

Ensures MAR has been initialed and Daily logs are completed daily.

All other duties as assigned.

Requirements:

Demonstrate strong written and oral communications skills.

Proficiently use Microsoft Office and Google Docs

Pass skills tests related to physical intervention.

Pass a Department of Human Services background check and provide two professional references.

Work independently and have strong organizational skills.

Required Experience/Education:

2 years experience working with individuals with high behavioral support needs preferred.

Minimum of 1 year of DC, Program Management/Supervisory experience, preferred.

Bachelors degree in a field related to human services, preferred.

245D compliance must meet one of the following:

A bachelors degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older.

Minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).

Associate degree in a field related to human services, and two years of full-time work.

Diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older; or

Strongly Preferred:

Current CPR and First Aid Certification

Ability to work on holidays and during inclement weather.

Ability to develop, implement and evaluate client programming.

Ability to problem solve and work with multiple teams regarding staff scheduling.

Knowledge of states and federal requirements as they pertain to the 245D regulations.

Strong written and oral skills

Available shifts and compensation: We have available shifts all days of the week. Compensation is $21.00 - $25.00/hour.
