Fraser is currently seeking experienced group home coordinators with strong decision-making skills and who are able to manage their time well to get tasks done accurately and efficiently. This position requires individuals to be self-motivated. Applicants with strong communication skills and experience coordinating services are strongly desired. We are currently offering a $750 Hiring Bonus.

Responsibilities:

In our Community Living program you will provide supervision and leadership to direct care staff working in our Supervised or Supportive living home setting. You will be providing direct cares to our clients and supervisory duties. You will comply with all regulations, policies, day-to-day operations, and budgetary parameters of Fraser's Community Living division. You will also develop and assure documentation of outcome-based recommendations, coordinated service and support plan addendums, and individual behavior/therapeutic plans for each client served, in addition to assuring accurate implementation of those plans.

  • The Designated House Coordinator position starts at $21--$24 (MN State Qualifications Apply-see below)
  • Weekend shift differential ($1.00 added to base pay rate for hours worked 10pm Friday through 9am Monday.)
    • If an employee works overtime in a week where the weekend shift differential was applied, the shift differential rate will be factored in to a blended overtime rate.

Locations and Schedules:

  • We have various locations across the metro with various schedules. Typically, Monday-Friday, business hours.
  • All schedules are fulltime with benefits.

Requirements for the Designated Coordinator positionare a combination of education and experience, as outlined by MN Statute 245D. Please read carefully after applying.

Candidates must qualify based on one of the following three:

  1. BA/BS in Human Services or related field with 1 year of full-time work experience(or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older.
  2. Associate degree in Human Services or related field with 2 years of full-time work experience(or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older
  3. A minimum of 50 hours of education and/or training related to human services and disabilities with four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older. Note: Supervision must be provided by a qualifying Designated Coordinator under 245D.

Additional Requirements for all positions:

      • Valid driver's license with acceptable record
      • Ability to meet DHS standards on criminal background study.

      • Excellent verbal and written interpersonal communication skills.
      • Basic mathematical skills with ability to accurately manage a budget and create financial reports.
      • Proficiency in using Microsoft Office programs including Excel, Word, and Outlook
