Job description

The Designated Coordinator will provide oversight, support, and evaluation of the delivery of services to persons served according to the coordinated service and help plan and addendum. This position will ensure quality service delivery and compliance with state regulations and oversee training and support of program teams to develop, implement, and conduct a customized plan that helps guide the person toward achieving their goals.

Qualifications: Designated Coordinators must meet qualifications in accordance with 245D requirements, which are:

  • (1) a baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  • (2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  • (3) a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  • (4) a minimum of 50 hours of education and training related to human services and disabilities; and/or
  • (5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).

Required Skills and Experience:

  • Supervisory and/or leadership experience in a social service organization
  • Effective oral and written communication skills
  • Experience with the administration of 245D licensing requirements
  • Reliable transportation for work driving purposes, valid driver’s license, proof of valid car insurance, and permit driving record checks as needed
  • Leadership and management skills including time management, organization and prioritization skills.
  • Receptive to learning and applying new skills, techniques, and approaches
  • Demonstrated flexibility, anticipate and respond effectively to change
  • Ability to consistently exercise discretion and independent judgment

Responsibilities:
Supervision of program staff including but not limited to:

  • Create and maintain staffing patterns
  • Provide ongoing performance management, including 1:1 check-ins, performance reviews, and performance improvement as necessary
  • Participate in direct service professional development, including but not limited to designing and delivering monthly training as needed
  • Conduct staff orientation training to ensure competency working 1:1 independent of supervision when working with vulnerable adults"
  • Attend meetings and facilitate communication between the management team and program staff

Oversee work efforts and training requirements to ensure 245D compliance including but limited to:

  • Ensure 245D annual training requirements are met by all direct support staff
  • Provide staff training on shadow shifts, client service requirements, support plans, and person-centered planning
  • Ensure compliance with 245D.07 Service Planning and Delivery and 245D.071 Service Planning and Delivery; Intensive Support Services
  • Ensure high quality documentation and performance by making sure that appropriate quality assurance measures are in place

Coordination of client services including but not limited to:

  • Conduct client intake, admission and assessments, and assure that individualized service plans are developed, implemented, reviewed, revised as necessary and reported on as required by licenser/certification standard
  • Attend client support team meetings as needed
  • Serve as a communication liaison between guardians and / or other service providers
  • Maintain annual and as needed Client Service agreement renewals
  • Responsible for Incident support and reporting
  • Maintain the 24-hour Emergency Assistance program and participate in on-call rotation
  • Help fill shifts if staff aren't available.

Coordinate program development including but not limited to:

  • Responsible for ensuring programs are run according to the organization’s mission and promoting a positive work environment
  • Develop and maintain program policies and procedures, keeping up-to-date with state rules and legislative changes for all types of services
  • Develop and maintain strong community relationships through working with counties and regions to identify key need areas for new program development in response to community needs
  • Conduct program evaluations and quality assurance reviews, including client file audits, documentation audits, and assessment audits

Administrative duties include but not limited to:

  • Maintain monthly client summaries and send to case managers
  • Ensure compliance deadlines are met and document all meetings, reports, and assessments
  • Complete weekly dashboard metrics and designated coordinator checklist
  • Participate in management meetings and organizational planning
