Summary/Objective
A Community Support Lead will provide leadership, and support to the direct support staff, and ensure that quality care is provided to individuals. He/she will participate in activities that support individuals independence; monitor the performance of all DSP in the facility, provide feedback to supervisors as needed; to ensure that all documents are appropriately filled out and completed. They will manage household and resident funds; keeping a check on expenditures; assisting with emergency situations as needed and will complete all other duties as required.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Monitors all program aspects to ensure the highest quality of support and services are provided effectively and efficiently.
- Ensure proper handling of household & oversee each persons financial expenditures.
- Ensured implementing and initiating active participation in activities of daily living (ADLs), community integration and inclusion.
- Organize, facilitate, and/or follow-through with peoples recreational and social activities in accordance with peoples preferences.
- Establish and maintain good relationships with all.
- Implement Judaic programming in the home including following rules of kashrut per JFGH standards.
- Function as a liaison with residents families, day placement staff, program managers, and other Interdisciplinary Team members.
- Document all contacts and necessary communication:
- Documenting outings and activities.
- Completing emergency incident reports.
- Completing medical record.
- Documenting communication in the systems diary notes.
- Review and facilitate persons goal data based on their persons centered plan.
- Collecting behavior plan data, as required.
- Reporting all house and vehicle maintenance needs in a timely manner to the property operations team via system.
- Ensures all facets of care and treatment necessary to provide quality services to the people we support of assigned community residential setting in accordance with the person-centered plan.
- Assume primary role in supporting community residential individual achievement of Personal Goals as stated in their Individual Plans.
- Ensure timely follow-through on recommendations from the delegating nurse and other medical professionals.
- Accompany individuals to appointments and meetings.
- Plan staff schedules on the assigned home based on the funding ratio and person-centered plan, supervisor the staff on the ground while they perform their assigned duties.
- Fill the schedule in case of call outs by finding suitable staff using resources available.
- Mentor newly hired DSPs as assigned
- In collaboration with the Program Manager, makes decisions on recruitment/retention and disciplinary actions.
- Assist PM when requested with the completion of payroll.
- Complete performance evaluations for DSPs in coordination with the PM
- Conduct staff meetings.
- Must be aware of and ensure compliance with relevant policies, regulations, and statutes per federal, state, and local entities which include COMAR, State of Maryland Developmental Disabilities Administration (DDA), Office of Health Care Quality (OHCQ), Montgomery County Government, Medicaid Home, and Community-Based Services Settings Rule and JFGH Policies and Procedures, Standard Operating Procedures, and instructions from the supervisor.
- Function as assigned and maintain familiarity, in accordance with federal, state and county Regulations, for the people we support in JFGH, as assigned.

To be Makom’s next Community Support Lead, you will need:
- High School Diploma (required).
- 2 years of proven experience in Human Services, with individuals with IDD.
- Excellent Communication skills
- Proficiency with Microsoft applications & computer skills
- Ability to multi-task, work independently and with others, exercises professional judgement.
- Valid US driver’s license
