Job SummaryJoin us to reshape the future of chronic disease management! We’re seeking a passionate Operations Assistant to join our Unified Care team with both technical and Clinical Operations roles to develop an innovative, team-based chronic disease management program. You will utilize iHealth devices and Unified Care platform to help manage patients with chronic conditions on-site and online with other clinical team members.
Under the general supervision of a professional Clinical Operations team, the Operations Assistant will assist with direct patient care and program operations as dictated by established protocol.
Primary Duties And ResponsibilitiesThis role requires a combination of skill sets of patient care, Clinical Operations, account management, and business development. You will lead the patient enrollment and account management for iHealth Unified Care value based projects in collaboration with medical groups, and assist with the future business expansion in each physician practice.
This role also requires you to be tech-savvy, and you must be comfortable with working with our internal messaging (Google Hangout, Zoom, Google spreadsheet, pdf, printers, etc. ). Also requires you to be patient and able to explain technology to a non-tech savvy population. You will be responsible for teaching our elderly patients on how the mobile app and medical device works, as well as helping them with troubleshooting any tech issues.
The account management key responsibilities include:
Convince physician practices to sign up for the value based care projects led by medical groups under the supervision of Sr. business operations manager and regional account manager.
Set up the customized workflow for patient enrollment and care coordination with physician’s care team in each physician practice.
Coordinate with Medical Groups’ care quality team and the physician practice’s staff to achieve the patient enrollment goals.
Travels are required to provide onsite patient support in different locations in the San Francisco and East Bay area in Northern California, and short term travels to other areas for new team set up & training as needed.
The patient care key responsibilities include:
Reach out to patients and introduce the iHealth Unified Care program and services.
Teach patients to use iHealth Unified Care mobile app and devices to monitor their vital signs at home and communicate with our care team members.
Obtains pertinent labs and other tests as needed by the provider.
Maintains accurate documentation in electronic health records.
Manages clinic inventories and medical supplies for the iHealth Unified Care program.
Educates and advises patients on specific medical issues within established parameters to include obtaining prescription information.
Practices safety, environmental, and/or infection control methods.
Collaborate well with teammates, providers and staff in clinic for patient care.
Bachelor's degree required
Excellent communication and patient care
Excellent interpersonal skills
Experience working with patients in a clinical setting
Essential data analysis skills using tools like Excel or google sheet
Ability to perform standard clinical procedures according to established protocols
Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
Ability to maintain quality, safety, and/or infection control standards
Must have basic keyboarding and Windows computer skills
Must be a team player
Experience working with patients with Diabetes and Hypertension in a clinical setting is a plus
Registered or Certified Medical Assistant is a plus.
Flexibility to commute as needed
Bilingual is highly preferred, at least one of these: Mandarin, Spanish, Cantonese, Vietnamese
