Acute Care Coordination is a collaborative process of assessment, planning, facilitations, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality and cost effective outcomes. Provides Social Work Care Coordination by coordinating appropriate resources and services for patients and families.
Job Summary
- Provides Social Work Case Management by coordinating appropriate resources and services for patients and families under the guidance of a MSW for 1 year.
Qualifications
Education
- Requires Bachelor’s degree (BSW) in Social Work; with student internship experience in acute care case management will be accepted in lieu of the MSW.
Experience
- Student Internship in acute care case management preferred.
(Above requirements can be met by the equivalent combination of education and experience).
Licensure / Certification
- Either degree must be from a university accredited by Council of Social Work Education.
- Licensure by Oklahoma State Board of Licensed Social Workers preferred.
- Basic Life Support (BLS) training or re-training is required during orientation prior to providing patient care.
- Current Basic Life Support (BLS) is required to be maintained thru re-training for duration of employment.
Compensation / Benefits
- $19.56-$31.94/h.r based on previous work experience
