We have an opening at our Tulsa office. The role of Housing Care Coordinator requires a Bachelor's degree in the field of social or human sciences from a recognized university.


Care Coordinator II: $49,201.67 annually

We are looking for a team player who takes initiative and is passionate about helping others. This position will provide Housing Case management services to clients who may be experiencing or are at risk of experiencing homelessness. This position will work to transition clients to transitional or permanent housing by connecting them with the appropriate community resources. This position will also play a key role in working with our clients housed in our transitional housing program. Duties include but are not limited to assessing housing needs and preferences, educating about housing options, navigating housing- related benefits, completing housing applications, support with housing interviews, advocating for housing placements, support with skills necessary to maintain housing, and developing and sustaining relationships with housing providers. The ideal candidate for this position would possess a high-level patience and flexibility, as well as a good working knowledge of housing resources in the Tulsa area. Candidate will be responsible for incorporating the agency’s Sanctuary Model principles and tools into client care. This position requires a bachelor’s degree in the field of social or human services from a recognized university. Must have or be eligible for Case Management II certification. Minimum of one-year experience working with adults in a mental health or related social service field. Candidate must have a valid driver’s license and reliable transportation.

Special Knowledge, Skills and Abilities:

  • Must possess good working knowledge of community social service resources, basic crisis management skills, behavioral health resources, ability to provide trauma-informed care and a commitment to the Sanctuary Model components.
  • Must always demonstrate professional and ethical behavior as well as the ability to maintain confidentiality guidelines for interactions regarding client information.
  • Basic computer knowledge and ability to use Microsoft software such as Outlook, Excel, and Word.
  • Ability to establish working relationships with clinical, administrative, and support staff as well as community partners to develop professionalism and coordination within the team.




Example of Position Responsibilities:

  • Provide housing support services which may include assessing housing needs and preferences, educating about housing options, navigating housing- related benefits, completing housing applications, support with housing interviews, advocating for housing placements, support with skills necessary to maintain housing, and developing and sustaining relationships with housing providers.
  • Provide services to clients in their homes, communities, and schools (if applicable) while incorporating the entire family and supports in the treatment program, with focus on their strengths as well as the client's age, developmental level, and cultural heritage.
  • To assist in preventing an out-of-home (or school) placement and/or in facilitating discharge from an inpatient or residential treatment facility.
  • Plan and coordinate case management needs, including referral and escort to medical, psychiatric, substance use, crisis intervention, legal, housing, education/employment, entitlements, social, and peer support services.


Other Duties: Perform other duties as assigned.


Physical Requirements: Must be able to move from location to location, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures/programs, enter data to various programs. Must be able to climb stairs and access non-handicapped equipped residences/facilities.
