CCBHC is an integrated community behavioral health model of care intended to provide integrated, evidence-based, trauma-informed, recovery-oriented and person-and-family-centered care. The CCBHC offers the full array of CCBHC-required mental health, substance use disorder (SUD) and primary care screening services. Work in partnership with others within Family & Children's Services and Community Providers as part of a structured inter-disciplinary team, within in a CCBHC model, to provide a set of social service functions that helps clients access the resources they need to recover. Responsible for providing outreach and engagement to clients in the community, individual, family and/or group services providing high quality targeted case management and care coordination services that will assist individuals in sustaining recovery, and gaining access to needed behavioral health, medical, social, legal, housing, educational, employment, and other services and supports including arranging, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring. Care is provided in a culturally sensitive way and is delivered community based to an adult population who carry a serious mental illness, substance abuse, co-occurring and/or medically co-morbid diagnosis. Case Manager will be part of an outreach team who's primary responsibility is to make contact with clients who have disengaged from outpatient services, meet their immediate case management needs, and assist them in reengaging in outpatient services with their primary treatment team. Majority of services will be provided in the community.

Pathways to Care Team provides intake, assessment, community outreach, engagement services, and transitional care services to individuals with severe mental illness. Clients served by Pathways to Care Teams are transitioning from a recent urgent recovery center discharge to outpatient services. Clients often have complex mental health needs, including acute psychosis, histories of trauma, and co-occurring substance use disorders. The primary function of Pathways to Care Team is to increase access to outpatient behavioral health services and help clients successfully establish care with an outpatient provider.

    • Part of a Pathways to Care CCBHC team with a focus on clients discharging from a urgent recovery center and transitioning to outpatient care.
    • Serve adult clients, age 18 and older, that are diagnosed with a severe mental illness, substance use disorder, or co-occurring disorder.
    • Combination of office, remote, and field-based services with potential transportation of clients using personal vehicles as determined by needs of clients and/or program.
    • Conduct outreach activities to engage clients who are difficult to find, and engage, with an emphasis on ODMHAS “Most In Need” and Pathways to Care client. Outreach Case Manager does not have a dedicated caseload but will have a fluid client list to outreach and engage in services. Outreach and engagement activities will occur to assist clients and families to access services, benefits, formal or informal services to address behavioral health conditions and
    • Provide care coordination with active client involvement through appropriate linkages, referrals, coordination, and follow-up to needed services and supports such with the treating physician, PCP or other qualified health care professional.
    • This care coordination involves not only referral but follow up after referral to ensure that services were obtained, to gather the outcome of those services, and to identify next steps needed.
    • Communicating and coordinating care with external health care providers (pharmacies, PCPs, FQHCs, labs, home health agencies, etc.) utilized by the client.
    • Monitoring and follow-up activities with treatment or service providers for the purposes of monitoring client attendance of scheduled physician, medication, therapy, rehabilitation, or other supportive service.
    • Ensuring clients are assigned to the correct clinical care
    • Provide transitional care services including transitions from urgent recovery centers, inpatient, residential, or crisis centers, as well as transitions between levels of care within the agency and/or transitions from different age groups.
    • Provide care coordination while the client is hospitalized as soon as it becomes known and go to the hospital setting to engage the client in-person and/or through telehealth.
    • Care coordination is the deliberate organization of client care which includes sharing information among all the participants concerned to achieve safer and effective This means the client’s needs and preferences are known ahead of time and communicated at the right time to the right people, and this information is used to provide safe, appropriate and effective care to the client.
    • Work collaboratively with team members to ensure that there is appropriate and consistent coordination of care across all disciplines, primary care providers and other community providers as applicable.

    • Provide services in-person, telephonic, or via telehealth
    • Meet clients where they are in terms of current mental health symptoms and treatment
    • Meet with clients where they are in the community, including homeless shelters, homes, or outdoors, promoting confidentiality in all settings.
    • Gather and complete all CCBHC data metrics and SAMHSA grant required National Outcome Measures (NOMS) data as needed.

Bachelor’s Degree in the social sciences field is required, with consideration given for degrees in Criminal Justice or Education. Case Management II certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS. Must have a strong commitment to the rights and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services. Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues and respect for client rights and personal preferences in treatment are essential. Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.

This job is classified as a “safety-sensitive” position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Due to the “safety-sensitive” classification, an employee in this position would be subject to drug and alcohol testing, including random testing. Marijuana is one of the substances included in the drug panel screening. Possession of a medical marijuana license will not excuse you from the testing process or the consequences of testing positive for marijuana per the Family & Children’s Services Drug Free Workplace Policy, including possible revocation of a job offer or dismissal from employment.
