CASE MANAGER in Hyannis, MA

NOTE: You must live on Cape Cod, MA--this position is on-site.

Sign On Bonus of $1000 ($500 at 3 months; $500 at 6 months. Must average 20 hours weekly minimum) It is a great time to join LIFE!

LIFE on Cape Cod has expanded with a NEW Community on South Street in Hyannis that needs 1 or 2 more staff to round out this developing program!!!

The CASE MANAGERS assist our residents in fulfilling their independent potential by assisting them in managing their daily life skills such as activities in the community, household management, menu planning & shopping, meal preparation, personal presentation and hygiene, appropriate behavioral and social skills, routine fitness, transportation needs, medical appointments, and access to recreational entertainment, etc.

Qualifications: PATIENCE, KINDNESS, DEPENDABILITY and a SENSE OF HUMOR and the ability to support these wonderful individuals to have productive and happy lives, lived as independently as possible!!!

Experience and/or education with our population (or a similar population) is desired. (professional, personal, or volunteer experience is valued)

To meet the needs of this unique community, the hours needed are:

• Shifts available fall between Monday-Friday 8AM-6PM (6-8 hour shifts ideal)
• Part Time-- we will be as flexible as possible to make a schedule that works for you!

The applicant must:

• Be creative and enthusiastic in creating activities and executing activities to enjoy with the individuals throughout the communities of Cape Cod.
• Must be a team player with the ability to work with the entire team to the benefit of the individuals we support and be an independent thinker as well
• Must have good organizational skills
• Must have excellent verbal and written communication skills and good computer skills
• Experience with Human Services, Learning Disabilities, Children, and/or Seniors is desired (Work experience, Volunteer experience, Special Olympics experience and Personal experience are all highly valued)
• An interest in the culture and activities in the mid-cape area and sharing that experience with the individuals in this community an excellent trait
• The confidence to help these individuals grow by watching and learning from your role modelling
• The ability to maintain the Human Rights of all of the individuals we support

Responsibilities:

• Assists, organizes, and facilitates activities as assigned/developed
• Assist the individuals in this community with meeting their goals and objectives in the following areas: apartment maintenance, personal health maintenance, nutrition, fitness and social skills development
• Assist with grocery shopping and preparing healthy meals safely
• Participate in team-related services; staff meetings, resident/roommate issues, resident case reviews with staff and/or family, etc.
• Complete routine documentation after every session or significant communication with residents
• Make recommendations to meet the needs of residents that are beyond the scope of the LIFE Coach (i.e., medication adjustments, therapy by licensed clinician, private tutor, house cleaner, etc.)
• Maintain sanitary conditions of workspaces and vehicles; complies with LIFE hygiene guidelines.
• Must have a valid driver's license with acceptable driving record and dependable transportation
• Must be able to pass CORI and fingerprinting background checks through our governing agency (DDS) and DPPC.
• Be willing to attend training through LIFE or have current certification in CPR/AED/First Aid
• Any other such duties as requested/required
