Hiring Case Managers in the following areas *Merrimack Valley-Lawrence, Lowell, Haverhill /*Boston-Dorchester, Roxbury, Chelsea, Roslindale/ *Northshore-Lynn, Salem, Peabody/*Worcester- preferably Vietnamese speaking needed for A busy Home Care Agency.

Required Education and Experience:Recent Graduates are Welcome!!

Valid License or Bachelors Degree for Social Work

Valid driver's license

Essential Functions:

*Conducts monthly home visits as needed including  initial assessment.

*Communicate with members to make sure they are happy and safe with their services provided in in their own home.

* Observe caregivers work and make sure all guidelines and tasks are being followed. 
*Complete and prepare all necessary documents, data, forms etc.. as required
*Keeps updated progress notes in accordance with EOEA documentation standards.
*Works closely with Director, Coordinators, Care Managers, Caregivers, Members 

* Recruit members to the agency
