SUMMARY
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Educates the community on various homebound meal outreach program and the importance of utilizing the program
- Distributes educational materials in the community
- Visits homes to determine client needs, develops plan to meet needs
- Identifies and monitors clients for homebound meal program
- Completes required paperwork in compliance with state and program regulations
- Administers initial nutritional screening in the form of questionnaire
- Helps client obtain necessary resources
- Refers clients with social and emotional problems to other community agencies for assistance
- Refers clients to Director or Nutrition Services for nutrition counseling as needed
- Documents client cases including assessment and reassessments and maintains records on each client
- Prepares reports as required by various federal, state, and local programs
- Helps organize and participate in fundraisers benefitting the program
- Other duties may be assigned

REASONING ABILITY
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
- Microsoft Word and Excel software
- Ability to learn new software - Serv Tracker
- Valid driver’s license required
WORK ENVIRONMENT
- While performing the duties of this job, the employee is occasionally exposed to fumes or airborne particles and outdoor weather conditions
- The noise level in the work environment is usually moderate
