Job Description:
- Assist clients with the application process regarding various benefits and services including food benefits, Medicaid, general assistance, childcare, employment, Identification, birth certificates, social security card, and housing.
- Follow up on the status of all applications submitted and report all information to the client’s assigned peer.
- Work in conjunction with all current and future Bridge Clinic locations, both permanent and mobile, as well as the encampment department.
- Submit a weekly case management data sheet to immediate supervisor.
- Update the Case Management Analysis Spreadsheet as applications are completed.
- 2 years of case management experience
- Experience within mental health or substance abuse field preferred.
