RN Case Manager - Day Shift
M-F (Hours: 8a-4:30p with call rotation every 7 weeks) No Holidays!

Christiana Care Hospital in Newark is looking for an energetic, motivated Registered Nurse (RN) Case Manager with experience in Transition of Care for an inpatient hospital.
The Case Manager will be responsible for Case Transitional Care Management to ensure patients with a high-risk medical condition receive the care needed immediately after discharge from a hospital. This includes functioning as a member of the interdisciplinary team, creating, implementing, and monitoring treatment plans to ensure safe, timely, and effective transitions throughout the care continuum and discharge planning.
CASE MANAGEMENT:
  • Identify patients with post-acute care, placement, and complex discharge planning needs based on a comprehensive assessment that includes physical and psycho-social factors/needs.
  • Anticipate, initiate, and establish a discharge plan for patients with post-acute care needs, collaborating with the physician, nurse, and other health care providers, the patient, their family/primary caregiver(s), third-party payers, and employer following established clinical guidelines, standards, and pathways.

UTILIZATION MANAGEMENT:
  • Review the admission assessment and collaborate with the primary nurse and other health care providers to ensure a multidisciplinary plan of care is in place to meet identified patient care needs and desired outcomes.
  • Identify system issues that serve as barriers to care. Participates in the development and implementation of strategies to remove barriers and facilitate performance improvement measures.

PERFORMANCE IMPROVEMENT:
  • Review patients' progress with members of the health care team. Monitors use of clinical pathways, verifying appropriate use and progress toward identified patient care outcomes. Monitors compliance with National Patient Safety Goals within the assigned unit.
  • Participate in monthly unit-based PI meetings to improve compliance with National Patient Safety Goals and JCAHO ongoing readiness efforts.
  • Assist with data collection and implementation of action plans for CMS, JCAHO, AHA Quality Initiatives, and CCHS-defined measures.

Education & Requirements
  • An active RN license in DE or Compact State is required.
  • BSN preferred or commitment to obtaining BSN within three years of hire date.
  • RN work experience in Case Management, Transitional Care, or Discharge Planning in an inpatient hospital is preferred.
  • RNs with 1-2 years of experience in an inpatient acute care hospital are encouraged to apply.
  • BLS preferred.
  • Case Management Certification (CCM) is required within 18 months of eligibility.
  • Complete 8 continuing education credits (CEU) per year in Utilization and/or Case Management.
