Position Description - Case Manager
Job Summary
The Case Manager provides intensive case management services to participants ages 18 years of age and older. The main outcome of this position is the recruitment, enrollment, and retention of participants into a comprehensive education & employment program. This is a full-time exempt position at an adult education and job training program that focuses on the unemployed and underemployed, including veterans, individuals with criminal backgrounds, and at-risk youth. Regular hours are Monday through Friday 8:00 AM – 4:30 PM. This position requires flexibility for special events or other agency activities.
Essential Duties and Responsibilities: These duties and responsibilities are performed on a frequent and recurring basis by an incumbent and include the following:
- Assist with the development and execution of new and expanded programs in a collaborative fast-paced environment providing quality user-centered services and programs that meet the needs of the community.
- Coordinate case management services for participants to reduce or alleviate barriers to active participation and maintain continuity in services.
- Conduct orientations, collect intake materials, and screen participants for Administer, score, and schedule related assessments.
- Create, review, and manage participant files to ensure compliance with funding guidelines:
- Collect, scan, upload, and file eligibility documentation.
- Document all communication with and on behalf of the participant via case notes.
- Interview students and evaluate immediate needs; identify and connect to appropriate support services.
- Develop Individual Service Strategies with students; monitor progress and update action plans at least once per month.
- Monitor and track student attendance and progress.
- Maintain regular contact with participants on caseload, and conduct outreach for those who need to be reengaged.
- Assist with employment, post-secondary, and internship placements and tracking (follow-up services are conducted for one year after exit).
- Monitor and track student performance/attendance (daily) and complete/issue report cards (monthly). Also, complete progress/case notes/certificates (and other relevant documentation) regarding student performance. Note: Attendance is recorded daily with monthly total calculations.
- Ensure an orderly program environment that exercises appropriate customer service.
- Maintain an active understanding of agency programs in order to translate programs to opportunities for support.

Job Requirements:
Education: Bachelor’s Degree in psychology, counseling, social work, human service, or related field and/or ten plus years or experience.
Experience: At least 3 years of experience in case management or similar field.

Pay: From $45,000.00 per year
