JOB SUMMARY
The Case Manager is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file with supporting documentation maintained in physical files. Responsible for coordinating case management, safe and timely release services, and ensuring timely completion of ORR case management requirements. The Case Manager will facilitate coordination between all disciplines within the program to ensure timely reunification and safety of youths. The Case Manager demonstrates knowledge of and familiarity with the Office of Refugee Resettlement (ORR) policy and procedures relating to the care and supervision of unaccompanied children (UC). This includes knowledge of legal background and authority governing the practice as related to working with unaccompanied children.
ESSENTIAL FUNCTIONS
- Conduct initial intake assessments when children arrive to facility, updating portal with client information and potential sponsor information, initial phone call to family in home country and potential sponsor is provided to youth upon 24 hours of arrival.
- Conduct interviews of family members, friends of family and/or sponsors to determine the integrity of the relationship, verify sponsors identity and verify information received from minor within 24-48 hours upon admission to the shelter.
- Determine options available for youth within 48-72 hours and proceed with the required documentation to reunify youth with family in home country or in the United States as deemed applicable.
- Determines children’s requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
- Conduct sponsor assessments, home country assessments, and work in guiding sponsor to complete documents, background checks, fingerprint appointments in order to complete the reunification process.
- Maintain updated record of all documents from the sponsors and upload into the portal within 24 hours.
- Coordinate with local pro bono attorneys for the timely provision of “Know Your Rights” presentations to youth and ensure youth signs the acknowledgment and receives a copy of the Legal Service Provider list and Notice to Juvenile Aliens in Federal Facilities Funded by DHS or HHS.
- Ensure the timely completion of all assessment including Initial Intake, Emergency Placement, and Preliminary Service Plans in accordance with CFRC, State, and Federal requirements.
- Ensure the timely submission of the initial Individual Service Plan due within 6 days of the youth’s arrival to the shelter and 31-day updates thereafter in accordance with CFRC, State and Federal requirements.
- Establish treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Ensure accurate and timely completion of documentation as required by state and federal regulations, to include ensuring that all services provided to youth are documented in the client record and required databases in accordance with state and federal regulations. Document all actions taken and contacts with youth, sponsor, and stakeholders in the form of progress notes as required. Document every meeting with youth and update into the portal within 24 hours of meeting.
- Complete and submit reunification packets for initial review to Lead Case Manager or designee.
- Submit completed reunification packet with appropriate referral made by Case Manager for the timely release of youth to designated ORR representative.
- Provide weekly face to face updates to youth and telephonic updates to family members/sponsor with documentation.
- Ensure the provision of two weekly telephonic contacts with family of origin, primary caregiver and/or sponsor.
- Facilitate incoming calls to minors with the appropriate family members and other approved caregivers.
- Facilitate attorney to client contact as requested by youth.
- Coordinate weekly treatment team meetings with representatives from all departments at the shelter.
- Maintain physical and web-based client files, document, login events, and assessments. File all documents obtain by sponsors in youths file, along with case notes and other assessments conducted.
- Close files for discharges and create new files for new intakes.
- Drive clients to facilitate program services, may include transportation to court appointments, attorney visits, other appointments as per contractual requirements as needed. May also include transporting youth within the United States if reunified.
- Complete the census detailing which children have been discharged and entered the facility as new intakes.
- Maintain communication with multiple entities such as home study providers, ORR representatives service providers and sponsors.
- Prepares children discharge/release by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources. Send discharge notification to ORR when a youth leaves the facility.

QUALIFICATIONS AND REQUIREMENTS
- Level: Bachelor’s Degree in Psychology, Sociology, Social Work, Clinical Counseling, Human Services or other relevant behavioral science required. Additional degree/course work in related fields desirable.
- 2 years of progressive employment experience as case manager.
- Equivalent Education/ Work Experience: Bachelor’s degree and 2 years of progressive employment experience with demonstrated case management experience under HHS and ORR programs.
- Bilingual (English/Spanish) – speak, read and write
Working Conditions/ Physical Effort
Work is performed mostly in an office setting. Some overnight travel, public speaking, evening and weekend work are requirements of the position. Regular computer works also required. While performing the duties of this job, the employee is required to stand; walk; sit; finger, handle, or feel objects, or controls; reach; climb stairs; balance; stoop, kneel, or crouch; talk and hear. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
