Position Summary: The Social Worker is responsible for directing the day to day support services of the hospice. The Social Worker will assist in directing, planning, coordination and evaluation of the services provided by the support staff of the Hospice Services to include Bereavement, Pastoral and Volunteers
Essential Duties:
- Assist in the training of the hospice support staff in care delivery to the hospice patient and family
- Identifying the patient and family's psychosocial needs and coping skills
- Providing interventions for specific psychosocial symptom relief as needed; enhancing the strengths of the family system
- Consulting, collaborating, and contributing to the development of a plan of care by the IDG for all patients on hospice service
- Assessing bereavement risk, and referring for ongoing bereavement services
- Providing holistic family-centered care across treatment setting and bereavement follow-up for high-risk family members/caregivers; screening for psychopathology and education and intervening as necessary
- Connecting the patient, family, and caregiver with community resources
- Documenting psychosocial assessment, goals, care provided, and patient/family response to interventions
- Involving the patient and family in the treatment plan
- Participating in in-service programs
- Maintains a high degree of confidentiality at all times due to access to sensitive information
- Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
- Follows all Medicare, Medicaid, and HIPAA regulations and requirements
- Abides by all regulations, policies, procedures and standards
- Performs other duties as assigned

Position Requirements & Competencies:
- Master's degree in Social Work from an accredited university
- Current unrestricted licensure in the state of Texas to practice as a MSW
- Minimum of three years working in a home health and/or hospice care environment preferred or five years of clinical experience
- Proven leadership skills and the ability to build a team approach to patient care
- Proven understanding of clinical assessment including use of Quality Assurance Program, community resources, bereavement and pastoral counseling
- Strong communication skills and interpersonal skills
- Computer proficiency in Microsoft Office
- Knowledge of home care operations desirable
