PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. As an LPN, you will support the delivery of healthcare by providing nursing care in our clinic environment. You will work collaboratively with a multidisciplinary team to welcome patients to a safe and inclusive environment and to engage patients in holistic care.

In this position, you will work specifically with our Behavioral Health/Psychiatric Nurse Practitioner, three days per week at West Side Clinic and two days per week at University Clinic. You will be an integral part of building our psychiatric care program in the Des Moines area. We have a fast paced, team-oriented work environment. Ideal candidates will have some psychiatric/Behavioral Health nursing experience.

What You Will Do

1. Documents patient care accurately and timely in the electronic health record. (Clinical documentation)
2. Facilitates the patient rooming and intake process and obtains accurate patient vital signs per established workflow. (Patient rooming, intake, and vital signs)
3. Performs point-of-care testing per CLIA guidelines and other established workflows. Ensures service orders are completed timely. (Point-of-care testing)
4. Administers medications, immunizations, and treatments per written or standing orders. (Medication administration)
5. Performs lab and diagnostic testing per written orders, explains testing procedure and preparation, and reports test results per procedure. Ensures test orders are completed timely. (Lab & diagnostic testing)
6. Completes timely prior authorizations and referrals per established guidelines. (Prior authorizations and referral management)
7. Answers and returns phone calls timely.
8. Prepares for exam/procedure and assists provider as requested.
9. Provides patient education appropriate to the patient’s age and identified learning needs.
10. Collaborates with assigned Care Team by participating in team huddles and pre-visit planning for scheduled patients.
11. Completes and accurately documents quality controls per established process.
12. Performs pre-cleaning and transport of contaminated instruments per infection control policies and procedures.
13. Routinely evaluates supply levels in exam rooms, removing expired supplies and restocking as appropriate.

Qualifications You Need to Bring

• Completion of an accredited LPN program
• Current LPN license accepted by Iowa Board of Nursing
• Basic Life Support (BLS) for Health Care Providers Certification*
• Mandatory Reporter Certification*

*Must be obtained within Introductory Period if not current

Your experience and skills determine your base pay. The typical hiring range for this position is $19.00 - $23.00 per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $27.98 per hour.
