Position Summary:
We have an exciting opportunity to join our team as a Bed Coordinator.
In this role, the successful candidate will act as the primary facilitator of all patient transfers into NYU Langone Health facilities from outside hospitals, as well as all inter-facility patient transfers and direct admissions within NYU Langone Health, based on an established set of scripts and algorithms, in accordance with department protocols, policies, and procedures.
Job Responsibilities:
    • Demonstrates strong clinical assessment skills & clinical judgement to appropriately assess patient acuity and needs, assumes principal responsibility for the coordination of acceptance, admission and transfer, and necessary communication between the referring physician, attending physician and clinical staff.
    • Provides outstanding customer service/patient experience by answering calls in a friendly and positive manner, and resolves all inquiries in a timely fashion.
    • Monitors & documents all aspects of the patient transfer process. Obtains and documents/uploads all available patient medical information from referring facility (e.g. face sheet, lab results, E-MOLST, H&P, current medications, etc.) in the Electronic Health Record.
    • Facilitates on-line physician conference calls with Referring & Accepting Attending Physicians (and other key stakeholders, as necessary) based on service and level of care requested for all incoming transfers.
    • Appropriately prioritizes all incoming patient transfer requests in consultation w/ accepting/referring physicians, CTC Medical Directors, and in accordance with department policies & procedures.
    • Initiates the financial clearance process via Revenue Cycle Operations and/or Patient Access, and coordinate with referring MD offices or the referring Case Managers to obtain insurance authorization for patient transfer.
    • Serves as a primary liaison throughout the patient transfer and direct admission process for patient representatives, NYU and external physicians & physician extenders, nurses, EMTs/paramedics, third party payers, and other agencies as necessary.
    • Submits patient transfer request report w/ all pertinent clinical information to the appropriate CTC Medical Director (and to other key stakeholders as indicated) for review and transfer approval.
    • Confirms patient acceptance with accepting/admitting attending physician when according to acceptance guidelines for each respective NYU Langone Hospital facility.
    • Collects & documents patient clinical updates & current vital signs from the patients nurses/providers at referring facility throughout transfer process at regular intervals in accordance with department policy.
    • Coordinates requests for inpatient bed assignments for incoming transfers in collaboration with patient placement / bed management teams at each respective NYU Langone Hospital facility to ensure proper patient placement.
    • Facilitates nurse to nurse patient handoff/report via conference call for all incoming transfers.
    • Arranges and coordinates hospital to hospital EMS transportation arrangements in partnership w/ outside organizations, and communicates all pertinent transport updates (including estimated time of arrival) to the accepting/admitting attending physician and other key stakeholders as indicated.
    • Arranges clinical accompaniment by a MD, RN, or other clinician during EMS transport as indicated for critically ill patients.
    • Monitors Electronic Health Record system to ensure safe patient arrival at destination.
    • Identifies & immediately escalates any/all clinical issues, operational barriers, and/or safety concerns in real-time to CTC Leadership Team, Medical Director, and other key stakeholders as indicated.

Minimum Qualifications:
  • Bachelor's Degree in Nursing (BSN) from an AACN accredited nursing program
  • Active Licensure as a Registered nurse in the State of New York
  • Minimum 3+ years years acute care nursing experience
  • Must be positive, dependable, and have a demonstrated ability to work in collaborate, team-based work environment
  • Excellent organizational skills, and the ability to handle & prioritize multiple tasks simultaneously; Effective problem solving, issue resolution, negotiation & conflict management skills; Exemplary interpersonal & communication (oral and written) skills
  • Physical stamina, manual dexterity, visual & aural acuity to perform responsibilities

Required Licenses: Registered Nurse License-NYS
Preferred Qualifications:
  • Master's Degree in Nursing (MSN), health-related field (MHA, MPA, MPH) or business administration (MBA)
  • Emergency Department, Triage, and/or Critical Care nursing experience (strongly preferred)
  • Experience/familiarity utilizing Epic or similar Electronic Health Record; Technical proficiency in Microsoft Outlook, Microsoft Word, and Microsoft Excel

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $111,899.99 - $126,363.20 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
