Mds Coordinator

Position Title: RN – MDS Coordinator for a Skilled Nursing & Rehabilitation Center

Schedules Available: Full Time – Salaried position

Duties/Responsibilities:

· Overall responsibility for coordinating and completing the MDS’s in accordance with current rules, regulations and guidelines that cover resident assessments.

· Ensure that all assessments are completed and transmitted in a timely manner.

· Develop, implement and maintain an ongoing quality assurance program for resident assessments & care plans.

· Work with the interdisciplinary Care Plan Team to develop a comprehensive resident assessment and care plan for each resident.

· Reviews resident’s medical history, assist with resident interview to determine daily living skills, capacities and determine limitations.

· Participates in resident care conferences, discharge planning and utilization review meetings.

· Participates in resident meal pass, feeding and documentation of intake as requested.

· Maintain professional license and required ancillary programs (i.e. Infection Control Training).

· Demonstrate and maintain an atmosphere of teamwork and a positive attitude.

· Work in a professional manner with regard to residents, family members, visitors, and members of official agencies.

Requirements/Qualifications:

· Must have a current NYS RN License.

· Must have Long Term Care experience and at least one year of MDS experience preferred.

· Must be able to read, write clear and accurately, communicate and understand the English language.

· Must have patience, tact, enthusiasm and a cheerful disposition when dealing with residents, visitors, family members and other staff members.

Pay: $70,000.00 - $90,000.00 per year
