Job description

A bachelor’s degree in a human services field, health administration, business or public administration or closely related field and four years of administrative or clinical experience in a health or behavioral health program, one year of which must have included supervisory experience with oversight responsibilities for a health-related or behavioral health-related program. A master’s degree may substitute for an additional year of the general experience.
Preferred Qualifications: The preferred candidate would have experience in supervising and leading teams; experience with health and behavioral health services; familiarity with Adult Homes and services available under the Medicaid program; experience in housing and care management; experience working with older populations and/or populations with co-occurring needs; experience in managing and analyzing data; and experience reviewing health and/or behavioral health records.

Duties Description
The Department of Health (DOH) is seeking qualified candidates for the temporary, competitive positions (5) of Adult Home Project Coordinator to assist in the effective implementation of the Adult Home Settlement ensuring all remedy members have the opportunity to live in the community with needed supports. The Settlement requires the State to provide residents with serious mental illness living in certain Adult Homes in New York City (class members) with an opportunity to be assessed and to move to alternative community housing with support services if they wish to move and qualify. Implementation of the Settlement requires regular reporting to the court, counsel for the plaintiffs, and a court-appointed Independent Reviewer.
Under general direction of the Adult Home Project Director, the incumbents will work directly with and coordinate a provider team structure in a geographic region which includes housing contractors, care managers and peers that offer class members the opportunity to be assessed and to transition, with the goal of identifying, preventing and resolving barrier to transition. They will assist in the effective implementation of the Adult Home Settlement ensuring all remedy members have the opportunity to live in the community with needed supports. The incumbents will work in the field to ensure an improved experience of class members served under the Settlement and documents the compliance with the Settlement requirements. Travel between various provider sites is required. The incumbents provide support to the Project Director to facilitate working collaboratively with DOH and OMH.
Duties to be performed will include, but not be limited to, the following:
  • Oversees and coordinates the provider team in an assigned geographic region which includes: housing contractors, care managers and peers to facilitate transitions, monitor the progress and identify and resolve barriers to transition.
  • Ensure members of the provider teams are working cohesively.

  • Conducts on site visits to various providers.

  • Ongoing monitoring and evaluation of transition progress against the established benchmarks.
  • Provide training to providers on the use of cloud-based dashboard for tracking each case, transition related tasks and alerts for deadlines.
  • Monitor and track the dashboard to ensure cases are on track and following up with providers to resolve obstacles.
  • Look for patterns and trends that indicate broader issues requiring further investigation, such as a problem with a particular provider or Adult Home.

  • Assist in collection, synthesizing, analyzing and documenting data and information related to the class members served under the Settlement, with a particular emphasis on developing nuanced and qualitative descriptions of class members' decisions with respect to transitioning to the community by:
  • Reviewing class member health and behavioral health records including protected health information;
  • Travels to and meets regularly with housing contractors, peer coordinator, Health Home leads and care management supervisors;
  • Analyzing information about class member preferences, choices and experiences to identify trends and patterns;
  • Develop strategies to engage class members to offer the opportunity for assessment and transition;
  • Identifying issues impacting the ability of class members to remain in the community;
  • Collecting other data and conducting research as appropriate;
  • Generating case summaries, reports and presentations related to class member histories and experiences.

  • Provide technical assistance to provider team members to ensure transitions to the community setting are implemented in a safe manner.

  • Ensure new providers are incorporated into the team structure at each Adult Home, to further efforts to realign Health Homes and care management agencies.

  • Troubleshoots case-specific problems for class members in the community which may become a reportable incident and to ensure incidents are reported, reviewed and addressed.

  • Participate in interagency provider conference calls related to status updates on a particular class member’s progress.
  • Develop transition planning and evaluate post transition issues needing resolution.

  • Communicate weekly with OMH and DOH staff, other agencies and providers to track progress made.

  • Participate in quarterly leadership calls and meetings with OMH and DOH settlement staff.

  • Make recommendations for next steps and action items to the Project Director based upon data and discussion with providers.

  • Report back on additional analysis required as the result of meetings and recommend action.

  • Prepare documents and materials, including monthly and quarterly reports that demonstrate achieving improvements on process and transition metrics, housing placement progress over time.

  • Assist leadership with recommendations to inform changes to improve the experience of class members served under the Settlement and document compliance with Settlement requirements.
