GENERAL JOB SUMMARY:
The Admissions Coordinator is responsible for obtaining required information for the admission and discharging of residents through the Skilled Nursing Facility. This nurse is a member of the full SNF clinical team and may be assigned nursing staff or day to day clinical responsibilities. Complies with federal, state and local standards for community operations. Incorporates the Generation’s mission, vison and values.
SCOPE OF JOB TASKS:
Duties include but are not limited to the following:
- Greets residents and families upon arrival and answers any questions or concerns they may have.
- Completes admission notes, all initial vitals, and determines level of care, cognitive status.
- Processes all needed forms associated with admissions, transfers, and discharges.
- Provides clear communication with residents and families to ensure they feel informed and aware of needed details, polices, and procedures.
- Informs and instructs clinical staff and family members on patient’s care.
- Maintains strong working relationships with other staff members throughout the facility.
- Collaborates with other departments as it relates to the admissions, transitions, or discharge of residents.
- Keeps abreast of current Medicare/Medicaid regulations governing admission/discharge requirements of health care facilities.
- May conduct tours of facility for prospective residents.
- Performs other duties that may be assigned.
JOB SPECIFICATION:
Education:
- A minimum on one-year experience in Skilled Nursing Facility
- Ability to read, write, speak, and understand the English language
Skills, Knowledge and Abilities:
- Ability to read, write and communicate and follow written and oral instructions in the English language.
- Ability to communicate with empathy and compassion.
- This position requires the ability to speak in public and communicate clearly.
- Position requires the ability to make decisions using good judgment under stressful conditions.
Job Type: Full-time
Pay: $31.09 - $40.57 per hour
