Job description

This position will coordinate and conduct programming to participants of MHH, including volunteer training coordination and recruitment, and help with Peer Support training, as needed. The position will engage in assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet client needs. This position will maintain a focus on client self-determination and culturally relevant care, stressing personal responsibility and self-sufficiency. You will work under the direction and supervision of the Program Director.

Schedule: Monday - Friday 8:30 am - 5:00 pm
Duties
Direct Service Duties:

  • Create a warm and welcome environment for our participants;
  • Conduct and document intakes, screenings and needs assessments;
  • Advises of available programs and services, rights and responsibilities;
  • Provides information, linkages and referrals to community organizations;
  • Work closely with probation to process referrals and resources for CJ involved clients;
  • Coordinate and verify community services work for CJ involved clients;
  • Collaborate with the participant in goal planning to meet needs, identifying obstacles and setting goals and desired outcomes;
  • May perform field work and provide transportation;
  • Advocates for customers;
  • Provide service coordination and linkage including: Benefits enrollment, crisis intervention, vocational and employment assistance, general health and dental services, income support and benefits, substance abuse treatments, mental health services and consumer and family involvement
  • Assist participants in goal planning to maximize their progress toward independence and self-sufficiency
  • Participate in Care Coordination and Team meetings, as needed;
  • Collaborating with other professionals and support service providers across care settings, levels of care, and professional disciplines;
  • Coordinate interventions;
  • Identify barriers to client's engagement in own plan; addressing these barriers;
  • Group facilitation for Batterers Intervention, Child Abuse Intervention and Anger Management Programs, documentation and maintenance of client charts. Fee collection, records management, attendance records, and progress reporting. Maintain ongoing communication with supervising Criminal Justice agencies.
  • Conducts groups and classes based on created and approved curriculum and deliver with fidelity.
  • Advertises programming both in and out of county (Parole units, courts, probation offices, families and children's protective services, etc) and actively recruits participants;
  • Connects program participants with resources within the agency and externally;
  • Facilitate transportation for BIP, CAIP and AM clients, as needed. Provide other transportation, as requested;
  • Complies with all agency and program policies and procedures;
  • Attends regular program specific and agency-wide meetings and trainings as requested;
  • Prepare meals as needed;
  • Ensures documentation is completed in a timely and accurate manner;
  • Process volunteer applicants, provide volunteer trainings twice yearly, document attendance, maintain up-to-date volunteer agreements, job descriptions and training rosters, make recommendations to Director for program improvement;
  • Helps other Program Specialist(s) with Peer supporter training twice yearly;
  • Provide weekly posts to Heritage House Social Media (e.g., Facebook, Twitter) to include activities and events, calendars, and other program related news.

Indirect Service Duties:
  • Interprets, applies and follows applicable program policies and procedures;
  • Maintains case records, files and documents;
  • Provide information and recommendations to the Program Director regarding program evaluation and modification to better meet needs of clients, community and partners;
  • Prepare reports, as assigned;
  • Attend training workshops and updates as required by agency;
  • Participate in department staff meetings, case conferences and multi-disciplinary team meetings;
  • Bi-weekly Food Bank/shopping trips (served meals) for MHH, supplement food boxes, Senior apartments.
  • Prepare, submit and manage USDA paperwork
  • Organize pantry, prepare food boxes, pick up commodities
  • Other duties as assigned

Center Operations:
  • Answers phones and responds to requests for information/services;
  • Greet and checks-in with new participants as they enter the Center, introduce pet policy, verify pet vaccination records;
  • Shares in the maintenance of Center facility (cleaning and organization);
  • Communicate office supply needs to supervisor;
  • Document incidents through shift logs or identified methods

General Duties:
  • Assist in office coordination/organization and scheduling.
  • Provide coverage of office and fill in as needed.
  • Assists in the daily/weekly beautification and cleanliness of facility;
  • Daily record keeping and tracking through service logs, etc.;
  • Maintain open communication with team and community partners;
  • Attend staff meetings

Other Requirements:
  • Must complete finger printing and background check upon hire, at Alliance expense. Failure to pass fingerprint clearance or background may result in withdrawal of job appointment
  • Must have reliable transportation, a valid driver's license, and DMV clearance
  • The Alliance highly recommends vaccination against COVID-19. Staff are not required to be vaccinated but are required to show proof of vaccination upon hire should they have it.
  • Must complete annual TB Testing
