The Providence Center helps adults, adolescents and children affected by psychiatric illnesses, emotional problems and addictions by providing treatment and supportive services within a community setting.
The Community Support Program team is a Health Home team that treats adults with persistent mental illness to provide support and stabilization from an outpatient setting. The multi-disciplinary team is comprised of case managers, nurses, a clinical therapist, substance use specialists, and a psychiatrist.
Job Summary:
The Providence Center Unity Health Home Team - Registered Nurse works with team members to assess and evaluate client health care needs collected through interviewing, observation, record review and team input. Analyzes assessment data to formulate the basis of nursing care.
Identifies expected outcomes of care, individualized to the client, with the ultimate goal of improving clients overall health status. Administer medications per prescribers orders and monitor lab results as indicated.
Provides support to clients regarding daily living issues conducting home visits as necessary.
Collaborates with the staff members in providing care to clients. Maintains cooperative relationships with clients, staff, and the public.
Requirements:
Active RN licensure in Rhode Island.
Must have active driver's license and insured auto.
