Responsibilities
This is a traveling and telehealth position. The UAS Assessor Nurse will travel to Brooklyn, Queens, The Bronx, Manhattan, Nassau, and Westchester. On very few occasions, the UAS assessor may travel between two cities in one day, but that is not likely. The position is per-diem with the typical hours of 9 am - 5pm; however, there is flexibility with the hours.
Duties:
1. UAS Initial Assessments and Reassessments based on NY State requirements and guidelines and training provided. Perform remote telehealth and in-home evaluations of individuals seeking long-term care services and support. Evaluations include determining health status, strengths, and care needs.
2. Review consumer medical documentation and or health referral forms, as relevant to the case. Enter evaluation data into UAS-NY system and other EMR, as required.
3. Document any concerns, conflicting information, or other issues that surface during the evaluation process.
4. Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plans.
5. Discuss with a consumer on health care options, supports needed, service vendor options, and waiver options.
6. Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan.
7. Collect quality review data and any required documentation to support outcome measurements and record case notes. UAS Tasking Tool based on guidelines and training.
8. Responsible for correction and revision of UAS, paperwork, and tasking tool documentation based on review and feedback provided through quality and associated review processes.
Qualifications:
1. NYS is licensed and currently registered as a registered professional nurse.
2. Graduate of an approved school of nursing and holds a Diploma or Baccalaureate Degree.
3. Three to five years of satisfactory Nursing experience in Home Care (preferred). One year of UAS experience. UAS Assessor experience is a MUST!!!
4. Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.
5. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to speak effectively before customers/patients/members or employees of the agency.
6. Good Organizational writing, communication, and interpersonal skills.
7. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several variables in standardized situations.
8. Ability to solve practical problems and deal with various concrete variables in situations with limited standardization. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
