SalaryXincon Home Health care Services Inc. is seeking a dedicated and passionate Registered Nurse to join our team as a UAS-NY Assessment Nurse. The successful candidate will conduct patient assessment visits for Managed Long-Term Care (MLTC) plans via home visits using the standard UAS-NY procedures and protocol. As an UAS-NY Assessment Nurse, you will play an integral role in providing quality medical care to patients in need.
Qualifications:
- Associates Degree or above in nursing
- Active NYS Licensed Registered Nurse.
- Valid NYS Identification
- Demonstrated and literate computer skills
- Active CPR certification
- Excellent verbal and written communication skills
- UAS experience required
Key Responsibilities:
- Conduct initial and re-assessments for patients or current members in their home following the State's policy, procedures, and protocols
- Utilize UAS-NY/DOH tool to conduct assessments
- Complete required enrollment and re-assessment paperwork and use tasking tools to assist in determining eligibility for services

Pay Range:
- Initial visit is from $165 to $215 per case
- Reassessment is from $135 to $185 per case.
Pay: $100.00 - $200.00 per hour
