The Care Coaching Assistant 2 (Transition Coordination Support Staff) employs a variety of strategies, approaches and techniques to support a member's optimal wellness state by coordinating services & resources. The Care Coaching Assistant may support various functions including Transitions. Decisions are typically focused on interpretation of area/department policy and methods for completing assignments. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.Position Responsibilities:
- Contribute to administration of care coordination, primarily for transitions and custodial prevention teams.
- Provide non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.
- Perform varied activities and moderately complex administrative/operational/customer support assignments.
- Perform computations.
- Typically works on semi-routine assignments.

Required Qualifications:
- High School Diploma or equivalent
- Knowledge of Microsoft Office Word, Excel and Outlook.
- Minimum two (2) years administration support experience, preferably in a healthcare industry.
- Familiarity with care and well-being resources.
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences.
Preferred Qualifications:
- Associate's or Bachelor’s Degree
- Bilingual preferred
- Microsoft Access proficiency
- Previous experience working with Medicare, Medicaid and Dual eligible populations
- Experience providing administrative support in a healthcare setting with older adults or persons with disabilities.
