Position Summary:
Responsible for providing mental health and substance abuse treatment services to patients in a new telehealth outpatient setting. He or she will provide individual, family and group therapy, collaborate with the clinical team in the formulation of proper documentation and assist in coordinating discharge planning.
Licensures and/or Certifications
Active LCSW, LMFT, LMHC license is REQUIRED in the state of Florida.
Education and Experience:
Graduate from an accredited college or university with a minimum of Master’s Degree in Psychology, Social Work, Counseling and/or health-related field.
Telehealth Experience, Hourly rate $85.00 - $100.00 per hour
Position Responsibilities:
- Responsible for completing initial meeting to assess all patient/clients admitted to the telehealth program.
- Responsible for completing biopsychosocial assessment on all admissions within the organization prescribed time. Biopsychosocial contains all required elements.
- Completes/completing all clinical documentation on time and with all required elements.
- Facilitates and/or conducts individual, group and family therapy activities.
- Assesses discharge planning needs upon admission, throughout treatment, and during discharge planning process.
- Documents in the medical record any counseling, education, services, individual and group sessions, family interactions, assessments and reassessments, discharge planning, and any other intervention within the prescribed by the organization.
- Completes ASAM assessment upon admission, change in level of care, and on discharge. Utilizes ASAM assessment to determine appropriate placement.
- Completes suicide risk assessment as needed utilizing evidence bases, standardized tools.
- Completes client evaluations utilizing ASI, DSM-V or other diagnostic, standardized, evidence-based tools.
- Develops initial and comprehensive treatment plan based on interdisciplinary assessments and other diagnostic findings.
- Conducts interdisciplinary or other treatment team meetings on a regular basis to monitor client progress, achievement of treatment planning goals, treatment issues, discharge planning needs, and other issues.
- Provides case consultation as needed.
- Involves family in treatment as needed. Supports family program as needed.
- Participates in crisis intervention actions as needed via phone and/or video sessions.
- Supports and helps develop the patient/client treatment schedule. Monitors patient/client staff with assigned treatment schedule.
- Prepares and documents discharge summary.
- Obtain records or prior treatment episodes as needed.
- Assists patients/clients in considering and/or accessing programs such as Family Medical Leave, disability, and other services that may support community reintegration, discharge planning, and return to work.
- Assists in referrals to 12 step and other community programs.
- Responsible for demonstrating competence is at least one or more of the following: Establish rapport, systematically gather data, determine the readiness of the individual for treatment and change, and apply accepted criteria for diagnosis of substance use disorders.
- Screen for psychoactive substance toxicity, intoxication, and withdrawal symptoms.
Screen for co-occurring mental health issues.
- Able to support the organization’s efforts to comply with National Patient Safety
