We are looking for an experienced Support Coordinator professional (must live in Banks County, Forsyth County, or Fannin County, or any surrounding area) in the field of intellectual and developmental disabilities; and who have specific training about the services available in the Georgia state's Medicaid waiver programs. The successful candidate will be responsible for identifying, planning, and managing specific services that an individual will need to live a healthy, safe, and fulfilling life at home and in the community. The candidate will also provide a wide range of services, including customer service, problem solving, data entry, and administrative tasks. The ideal candidate will have the following skills:

Responsibilities:
- Develop and maintain relationships with individuals and other stakeholders.
- Conduct site visits to homes and community agencies
- Facilitate the service planning process and individual service plan development, and other meetings
- Assist with finding providers, providing information and referrals for community services, and monitor individual's on-going needs and quality of services
- Provide support and guidance to individuals.
- Monitor client progress and provide feedback to relevant stakeholders.
- Maintain accurate records of individuals progress and activities.
- Provide crisis intervention services as needed.
Job Type: Full-time
Pay: $45,000.00 per year
