Key Responsibilities:
- Provide comprehensive case management services to senior residents, including assessment, planning, and implementation of care plans.
- Collaborate with interdisciplinary teams to ensure the holistic well-being of residents, addressing their physical, emotional, and social needs.
- Advocate for residents' rights and facilitate access to resources and services within the community.
- Conduct regular assessments and reassessments to monitor residents' progress and adjust care plans accordingly.
- Offer counseling and support to residents and their families, promoting empowerment and resilience.
Qualifications:
- Master of Social Work degree from an accredited institution.
- Minimum of three years of experience in case management, preferably in a senior living or related setting.
- An adaptable problem-solver with strong critical thinking skills capable of thriving in dynamic environments.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with residents and families.
- Proactive, with a strong sense of self-motivation and accountability.
Preferences:
- Background in geriatrics
