Supports activities of the Community Practice Center by providing counseling, education, case management, and care coordination activities for patients and their families, including psychosocial assessment; counseling; patient education; referrals to social services and county medical assistance offices as well as linkages to mental health, substance abuse and primary care services as needed. Coordinate with various hospital services and other professionals in the provision of services and treatment plans. This position will lead an effort to screen all primary care patients for social determinants of health using validated screening tools. Develop workflows and streamline resources for individuals who screen positive for social insecurities.
This position requires providing services for adults and their families, in a manner that demonstrates an understanding of the functional/developmental age and biopsychosocial needs of the individuals served.
*This position is a 64 hour biweekly position, which is still Full Time*
Qualifications
Education/Training/Experience:
MSW preferred. 2-3 years experience in community, clinical or medical setting preferred.
Other: Must be willing to provide education to residents about social impacts on health and disease.
Licenses/Certifications:
PA Licensure preferred.
Conditions of Employment
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
