Concordia Visiting Nurses is looking for a full time Medical Social Worker (MSW) to cover our Southern and Northern service areas! This position is no weekends, no holidays, and no on call! Your patients, your schedule!

POSITION SUMMARY

The Medical Social Worker (MSW) is a qualified professional who provides Medical Social services to patients in the home as prescribed by the attending physician. The Medical Social Worker (MSW) is a member of an interdisciplinary team.

QUALIFICATIONS

• Master's or Bachelor's degree from a school of social work approved by the council on Social Work education. License by state preferred.
• One year of social work experience in a health care setting.
• Reliable, independent means of transportation to make home visits. Driver must have valid driver's license and drive a car with insurance limits as established in personnel policies.
