Job Description:
PURPOSE OF JOB:
To assist patients and families to identify social, emotional, and environmental needs and to connect them with available resources and services through the provision of a full spectrum of social work services. Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services. Represents, coordinates and implements Cancer Institute initiatives (educational, support, fundraising and programmatic) on behalf of the department.
ESSENTIAL FUNCTIONS:
- Actively designs, participates in and supports departmental and interdepartmental quality initiatives and goals, in order to best serve our patients.Identifies needs and opportunities for improvement as well as initiates, conducts, monitors and reports studies/outcomes on behalf of Cancer Institute. Including taking an active role in ACoS-COC and NAPBC Committees and survey preparation.
- Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive services.Able to use critical thinking and problem solving skills to determine how best to use available financial resources to assist patients in serious situations.
- Serves in leadership role within the Cancer Institute to develop and maintain educational and preventative programs/services identified as mission critical by physicians, hospital administration and the community. Works with Cancer Team to develop and initiate innovative patient programs.
- Provides psychosocial health and distress screening as a critical first step to providing high-quality cancer care. Collaborates with nursing to provide a comprehensive care summary and follow-up plan for patients completing their first course of treatment.
- Provides clinical social work services and counseling to patients, families, and significant others served in the Doylestown Hospital Cancer Institute. Coordinates outpatient programs and services in the Cancer Institute that meets the needs of patients.
Job Qualifications:
QUALIFICIATIONS:
- Education: MSW or MSS required, Oncology Certification preferred.
- Experience: 5 years healthcare related social work experience required, oncology social work experience preferred.
- Other Skills: Computer skills.
