Responsibilities:
-Manage and maintain relationships with clients, families, and external service providers - Regularly meets with participants to review case plan/goals
- Provide support and guidance to individuals
- Conduct assessments and develop individualized service plans
- Monitor progress and make necessary adjustments to service plans
- Collaborate with other professionals, such as social workers and behavioral health specialists, to ensure comprehensive care
- Maintain accurate records and documentation of client services
- Assist with budgeting and financial management for program activities
Experience:
- Experience working with individuals with developmental disabilities or related populations
- Knowledge of social work principles and practices
- Familiarity with budgeting and financial management processes
- Excellent organizational and time management skills
- Strong communication and interpersonal skills
