Service Coordinator

Full Time (40 hours per week) – Remote, Work from home

Make a difference every day by working one-on-one with older adults to coordinate services that help them remain in their own homes.

Primary Responsibilities:

• Perform screening & assessment of clients.
• Develop a plan of care.
• Identify long term services and supports
• Authorize services, collect, record and data.
• Provide follow up and monitoring of client visits.
• Help in individuals to remain safely their home.

Required Qualifications:

· Registered Nurse with current Licensure or

· Bachelor’s degree in Social work or related field.

· At least 2 years of experience in long-term care gerontology and/or care coordination is required

· Experience working with MS Word, Excel, and Outlook

· Ability to travel in assigned region to visit members in their homes

- Applicant will need to live close proximity to client service area which includes Hall and Howard Counties in Nebraska.

Preferred Qualifications:

· Medicaid Waiver experience

· Case Management and/or Care Coordination experience

· Behavioral health experience

· Field based work experience

· Experience with electronic charting

· Experience in long-term care, home health, hospice, public health, or assisted living

· Background in managing populations with complex medical or behavioral needs

· Knowledge of community resources

Job Type: Full-time

Pay: $21.00 - $22.00 per hour
