RN/Patient Care Coordinator

Overview:
Hospital for Special Surgery, #1 in Orthopedics for the last 14 years, is currently looking for an experienced and energetic Full Time RN/ Patient Care Coordinator for one of our physician offices. Dr. Nwachukwu, is an attending orthopedic surgeon at Hospital for Special Surgery, specializing in shoulder, knee, and hip surgery, and an assistant professor of orthopedic surgery at Weill Cornell Medical College in New York City.
In this role the RN/Patient Care Coordinator will be responsible for maintaining and improving the clinical and administrative workflow of a busy surgical orthopedic practice.
Your role is essential to promoting a professional atmosphere through courteous communication, cooperation and respect for patients, visitors, and members of the healthcare team. In addition, you will be responsible for upholding stellar customer service practices for patients and families, as well as acting as a liaison to the physician and hospital departments.
DUTIES & ESSENTIAL JOB FUNCTIONS
Patient Care Coordinator:
- Triage phone calls and patient inquiries, in a professional and efficient manner
- Retrieve messages frequently from voicemail (and EMR system where applicable) and return messages by the end of business day
- Serve as back up to Surgical Coordinator: Schedule surgeries and other ancillary tests as directed by physician or office manager
- Facilitate communication between physician and hospital departments, department management and patients
- Work independently on assigned tasks with minimal supervision
- Promote cooperation through teamwork
- Complete all in-service learning requirements including but not limited to the annual in-service learning.
- Perform other duties as assigned.
RN CLINICAL WORKFLOW RESPONSIBILITES
- Enters patient prescription requests in Epic for fulfillment by the physician
- Performs patient call backs for tests results as per physician instructions
- Triages patient phone calls and responds to clinical concerns as needed and per physician instructions.
- Maintains appropriate level of competency in using information systems.
- Participates in providing patient care and treatment in all Hospital settings, and alerts the physician to issues concerning patients' progress.
- Utilizes principles of infection control and standard precautions in the performance of patient care activities.
To qualify you must have bachelor’s degree and NYS RN license required. Minimum of 1 year RN experience required, as well as knowledge of surgical coordinating/OR booking. Familiarity with medical terminology and general office protocol required.

Develops and implements methods to continuously assess and improve department's performance.
Exercises Leadership:
Coordinates and integrates services within department and with other departments. Analyzes, evaluates, and solves inter and intra departmental problems.
Motivate personnel to achieve goals and objectives of the department and hospital.
Effectively coach employees, as per hospital policy. Provide orientation, in-service training, and continuing education for all persons in department.
Directs and oversees work of employees, including delegation of responsibilities, scheduling and assigning employees with minimum overtime usage.
Fire Safety:
Provides timely documentation and Summary Reports, ensuring all staff members demonstrate competence in Fire/Life Safety. (4 = 100% compliance plus demonstrated efforts to reinforce training; 3 = 100% compliance; 2 = 90% compliance; 1 = less than 90% compliance)
