We are currently looking for aPart TimeMDS Coordinator (RN) to join our passion team. If you are a Registered Nurse (RN) who has experience with MDS and who genuinely wants to make a difference for others, then we are eager to meet you!

Requirements:
- MDS Experience in a Long-Term Care Setting Preferred.
- PDPM experience required
- Ability to conduct and document accurate assessments of patients
- Ability to interview patients and families to ensure an accurate picture of the patient's psychosocial/medical status is obtained
- Ability to coordinate with other disciplines to ensure accurate and timely completion of the MDS.
- Organizational skills, ability to schedule and meet deadlines
- Monitoring the overall medical record to validate support of MDS coding
- Works with interdisciplinary team to assure timely completion of Resident assessment Protocols and Patient Care Plans
- Supports the accuracy of medical record documentation through ongoing in-services to center partners.
Successful candidates will have the following:
- Must possess a current, unencumbered license to practice as an RN in South Carolina (Required)
- Current CPR certification
- Experience with PCC
- Must maintain all required continuing education/ licensing and remain in good standing with the State Board of Nursing.
- Experience as MDS Nurse
- Expertise in long-term care/ rehab setting
The primary function of the MDS Nurse is to assess resident care needs, direct and supervise staff to meet the resident's needs, coordinate with other members of the Inter-Disciplinary Team (IDT) develop and implement a plan of care that meets the individual needs of each resident. The MDS Coordinator nurse also provides direct resident/patient care.
