The Resource Coordinator will also locate housing assistance and teach independent living skills to the consumers.
This is a community-based position, therefore the Resource Coordinator must possess the ability to work in a multi-disciplinary team.

Responsibilities/Duties:
- Collaborate with Case Managers and clinical team members to assess and identify consumer needs and participate in the design of interventions or action plans to fulfill the needs as they relate to community outreach.
- Collaborate with other outreach members to coordinate outreach activities, facilitate communication and to complete assignments.
- Attend and participate in regular site meetings by providing verbal updates related to community outreach.
- Facilitate/ coordinate support groups, health workshops or activities of educational consultants.
- Serve as advocate/liaison/broker for our patients with other community agencies when needed.
- Maintain a record keeping system for the purpose of tracking all services provided to patients and submit reports to Program Director as requested.
- Three (3) years community outreach work experience.
- 1 year of related experience preferred if applying to LA-ACT.
- High School Diploma/GED, some experience may be substituted at employer's discretion.
- Bachelor's Degree is highly preferred if applying to LA-ACT.
- Good organizational skills and ability to multi -task.
- Candidate must have knowledge of health care and other related resources both locally and citywide.
- Candidate must be able to work flexible hours as needed
- Valid Driver's License Required
