I) Job Purpose
The position of Recovery Manager will provide an intensive level of case management services to HCBS-AMH participants. This includes assisting HCBS-AMH participants in gaining access to needed, including Medicaid State Plan services; HCBS-AMH services; Medical services; Social services; Educational services; and Other resources. The Recovery Manager will develop and collaborate with contacts from MHMR Tarrant, Mental Health Clinics, and HCBS-AMH Provider Agencies.
II) Essential Functions
A. Employee processes and reviews all assigned cases.
Performance Requirement(s):

  1. Review all new referrals and contact the individual to introduce themselves as the Recovery Manager.
  2. Check the myAvatar system for current services and client data and information 100% of the time.
  3. Coordination with LMHA Point of Contact, HCBS Provider Agency, MCO, family members, other individuals to develop the client's IRP.
  4. Contact psychiatric service providers, family members/significant others, and/or other agencies to obtain information on client's psychiatric history, social functioning, and treatment needs.
  5. Obtain consents for release of information from each client referred or document the client's refusal to sign them 100% of the time.
  6. Inform supervisor(s) of clients who are not following the treatment plan immediately.
  7. Provide feedback to the IRP members, LMHA, Provider agency, psychiatrist, supervisor(s), judge, court staff, jail staff, and other MHMRTC staff and or medical staff as needed.

B. Employee provides case management, services coordination, monitoring services, and advocacy for clients.
Performance Requirement(s):
  1. Provide case management services to the client based on the individual needs of the client.
  2. Provide monitoring services for clients served in the programs.
  3. Provide face-to-face services based on the HCBS-AMH service protocol for clients served in the programs to ensure participation, promote adherence to treatment, and ensure the client's basic needs are being met.
  4. Follow-up within 24 hours with a client who misses a scheduled appointment with any MHMRTC staff person.
  5. Provide services in compliance with the Texas Resilience and Recovery Service Package requirements, including a plan to address housing and employment/income needs.
  6. Provide transition services for clients completing the program.
  7. Check medication logs, pills, etc. a minimum of once per week with each client to ensure medication compliance.

C. Employee reports and documents client services accurately according to the unit and agency procedures, and maintains working log of activity.
Performance Requirement(s):
  1. All ongoing contacts with clients must be documented using an approved progress note format.
  2. All progress notes for services must be written no later than 24 hours following the encounter with the client.
  3. Document through progress notes that a minimum of 45% of total work time is in direct contact with clients.
  4. Ensure that all client treatment plans consist of measurable goals and objectives that are agreed upon by the client.
  5. Ensure that treatment plans are reviewed every six months on all clients or as clinically indicated with 100% accuracy.
  6. Ensure that all paperwork is completed and kept in the designated areas for HCBS clients.

D. Employee produces positive outcomes as a result of their interaction with others.
Performance Requirement(s):
  1. Interact with clients, LMHA, HCBS-AMH provider agency staff, the MHMRTC organization, the Tarrant County court staff, and other organizations in a manner which promotes positive outcomes for the employee, the client(s), other staff, their supervisor(s), the MHMRTC organization, and other organization(s).
  2. Use channels of communication prescribed by the Agency Operating Procedure, established "chain of command" , laws, regulations, codes of ethics, professional standards, supervisory instructions, or other legitimate codifying entities in order to bring the client and system problems to the attention of the appropriate authority.
  3. An employee should have no more than three episodes per calendar year in which clients, other staff, the MHMRTC organization, and/or other organizations have had negative outcomes as a result of the employee's interactions with them.

E. Employee meets the required deadlines for completion of paperwork related to non-client aspect of job.
Performance Requirement(s):
  1. Completing all required documentation within a 24-hour period following service delivery.
  2. Turn in all mileage and timesheets by the required deadline to the Payroll Specialist or Assistant Director.
  3. Ensure that all credentialing requirements are being met to ensure that the employee maintains credentialing privileges.
  4. Ensure that all training is up-to-date to ensure that the employee is adhering to agency policy.

G. Perform other related job duties or responsibilities as requested or required, whether or not specifically mentioned in the job description.
  1. Exhibits a willingness to assume additional duties; seeks the guidance of supervisor prior to beginning an unfamiliar assignment.

III) Knowledge of Laws, Policies/Procedures, Skills, Education and Abilities
  • Knowledge of MHMRTC and BH Operating Procedures
  • Knowledge of community resources, including businesses and volunteer organizations
  • Federal Confidentiality Laws
  • Ability to communicate effectively in both verbal and written forms
  • Ability to establish and maintain positive working relationships with clients and staff
  • Working knowledge of psychopathology and DSM V classes of major mental illness.
  • Ability to work autonomously, organize work, and complete tasks assigned in a timely manner.

IV) Internal & External Customer Service
This position requires extensive internal and external contacts with all levels of management and field personnel. The employee will accomplish this with advanced written and verbal skills.
V) Travel
This position requires inter-agency travel, up to 75% of the time.

Additional Requirements
All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their departments as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.
This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and /or the Division/Director.
Assigned work hours may change as the needs of the agency change.
The functional Title of this position may change as the needs of the agency change.
All work will be complete within the scheduled work hours. All non-exempt (hourly) employee are expected to clock In and out for each work shift, no work done should be done off the clock.
Prior approval from supervisor is required for all Paid Time Off (PTO) and Overtime.
MHMR reserves the tight to change, and to or eliminate a position as it deems appropriate.
Employment is at will, as well as agency needs may change.
Agency dress code is to be followed at all times.
Physical on-site presence, including regular attendance and punctuality is an essential function of this position. Any change or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
