MISSION:
It is the mission of the South Carolina Department of Juvenile Justice (DJJ) to impact and transform young lives, strengthen families, and support safer communities through targeted prevention and rehabilitation.
JOB PURPOSE:
This position supports youth by creating and implementing sound re-entry plans, supporting re-entry initiatives, and assisting youth with successful re-entry into the community.
JOB RESPONSIBILITIES:
Assists in the development and implementation of individualized re-entry care plans.
Supports case management functions of helping youth identify goals, link them to opportunities and resources to aid in goal directed behaviors.
Adhere to the Integrated Treatment Model in terms of delivering treatment.
Conducts admissions/intake interviews.
Performs basic data entry and retrieval functions using an electronic management information system.
Coordinates transition and re-entry activities involving community Intensive Supervision Officers and external partners including outreach and support efforts.
Works with youth and youth's support network in order to help them access resources in their respective communities, and thus increase protective factors for the eventual return to their communities.
Coordinates transition services for youth leaving without probation or parole.
Networks and communicates effectively with a variety of diverse groups, organizations and agencies, including juvenile justice and social service professionals, law enforcement, courts, schools, employers, treatment providers.
Provides coaching, mentoring, and skill-building for youth re-entering the community.
Collaborates with Social Workers behind the fence providing crisis intervention when appropriate and communicate crisis conditions to clinical staff.
Participates in collaborative team meetings and trainings; prepares reports, chart notes, and completes data collection forms in a timely, accurate manner.
Communicates effectively both orally and in writing with staff and other collaborative partners.
Maintains appropriate client service standards in compliance with federal, state, and local laws and regulations.
Manages confidentiality appropriately; maintains appropriate and timely documentation.
Perform all other duties and tasks and required related to the mission of the agency.
Minimum and Additional Requirements
STATE MINIMUM REQUIREMENTS:
A bachelor's degree in guidance, counseling, or the social sciences and professional experience in human services or social services programs.
ADDITIONAL REQUIREMENTS:
Knowledge of services needed, interviewing and supportive counseling, community resources and programs, case management principles, objectives, and method, programs that address the youth needs, job market or job readiness programs, and family dynamics for successful placement.
Ability to effectively communicate with both the South Carolina Board of Juvenile Parole board and with SCDJJ Internal Release Authority.
Ability to effectively work with youth and coworkers in all agency divisions.
Knowledge of agency policies, practices, and procedures. General understanding of the SC Juvenile Justice System.
Under the direct supervision of the Re-Entry Coordinator, responsible for implementing successful re-entry initiatives supporting youth and families.
Must be able to work in a secure facility and the confidence and endurance to work in a chaotic environment.
Work hours may vary depending on the needs of the youth.
This performs job duties relating to the custody, control, transportation, or recapture of youth within the jurisdiction of the Department of Juvenile Justice and the employee will have direct and indirect contact with juveniles within the jurisdiction of the Department of Juvenile Justice. Training applicable to these duties will be provided.
