Position: Program Coordinator

Program: Community Health Programs

Department: Health Services

Reports to: Trans Wellness Center Site Manager

Compensation: $70,555 Annually

Status: 100%, Full-Time, Exempt, Regular

Probation: 180 days

DEPARTMENT SUMMARY: As the nation’s largest and most experienced provider of LGBT medical and mental health services, we help LGBTQ people lead healthier, happier lives, by keeping their minds and body well. All services are free or low cost. We’re also one of the few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research team working to advance the care and treatment of people in our community.

JOB SUMMARY: Under the supervision of the Trans Wellness Center Site Manager, the Program Coordinator will coordinate, schedule, and conduct activities and community events to specifically target transgender, queer, gay and bisexual men of color; Promote and provide referrals to STD/HIV counseling and testing; Coordinate activities that will lead to linkage to medical care for HIV-positive and PrEP eligible patients at Trans Wellness Center or any of the Center’s health centers; and assist with preparing reporting for community outreach, engagement, and marketing activities conducted under prevention program.

ESSENTIAL FUNCTIONS:

• Coordinates, schedules and conducts behavioral health intervention activities and engagement that help reduce the incidence of HIV rates among transgender, queer, gay and bisexual men who have sex with men of color.

• Facilitating weekly group meetings.

• In partnership with the Outreach Coordinator, oversees the development and directly implements community outreach and engagement activities to recruit into medical, mental health services and programs.

• Keep monthly calendar to coordinate, schedule and conduct activities at locations such as bars, bath houses and sex parties where transgender, queer, gay, and bisexual men of color congregate to assess individual risk behaviors.

• Develop and maintain a working relationship with other community-based organizations and business owners where outreach, workshop, and support group activities can be conducted.

• Monitor and report program goals and objectives.

• Coordinates and tracks budget planning and spending for program activities and engagement events.

• Assist with preparation of monthly quantitative and qualitative reports.

• Participate in community advisory board meetings, public task force and HIV planning meetings as necessary.

• Collaborate with local agencies that conduct HIV risk reduction programs targeting transgender, queer, gay and bisexual men of color and others who are at highest risk.

• Work collaboratively with the Nursing Manager and Clinical Services Manager to ensure the smooth flow of day-to-day operations.

• Coordinates financial screening visit, first medical visit, first psychosocial support visit, and other services such as mental health, 12-step programs, legal aid, housing assistance, general relief, etc. for HIV positive and PrEP eligible patients.

• Coordinates with Nursing and Psychosocial Case Management until patient successfully engages in medical care and ensures continuity of care following patient’s attendance of first medical visit and future medical appointments for a year.

• Must be innovative and a self-starter that is able to work in a constantly changing and fast-paced environment.

• Must be available to work some late evenings and weekends.

• Other duties as assigned.

JOB QUALIFICATIONS AND EXPERIENCE:

• Knowledge of and experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.

• A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.

• Successful experience with coordinating behavioral health intervention programs, including outreach, community organizing, and/or project management, a plus.

• Experience working in an administrative capacity (in non-profit organization/medical setting preferred).

• County certified HIV Test Counselor Certification and additional intervention program certification or willingness to obtain certifications.

• Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills.

• Ability to work independently in a fast-paced environment.

• Ability to handle and prioritize multiple projects.

• Strong Computer skills including word processing and other software systems preferred.

• Bilingual skills, a plus (English/Spanish fluency preferred).

• Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
