GENERAL SUMMARY:
Under general supervision and according to The Health Trust’s general policies and procedures, performs moderately complex duties to support the Meals on Wheels and Friends from Meals on Wheels Programs by performing a variety of duties related to client outreach, client friendly visits/calls, recruitment and support, with an emphasis on promoting customer satisfaction.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conducts outreach to enroll new clients in the Food and Nutrition Services programs.
- Provides support for Meals on Wheels food delivery, client onboarding, and customer service.
- Enters client information into the database and client tracking system and prepares reports as needed.
- Linkage to resources
- Works with the Program Coordinator to check client eligibility
- Prepares letters and other instructional materials for clients.
- Manage a client caseload of older adults and provide weekly phone calls or home visits to each client; or ensure that calls and visits are being made by a volunteer or intern. Calls and home visits may include: conversation, board games, reading, arts and providing support to the older adult in accessing medical and other community resources.
- Administer social isolation assessments and satisfaction surveys at regular intervals.
- Encourage participants to participate in the in-home exercise program.
- Facilitate group sessions, such as Televisits, for clients as requested.
- Contribute to monthly newsletter production and distribution
- Participate in outreach for new program participants and assist with the onboarding process.
- Accurately track and record services in the database.
- Become informed about resources in Santa Clara County for older adults, and demonstrate the ability to effectively discuss them and to share information about how to access them.
- Provides other client support services as needed.
- Answers phones, taking messages and resolving problems.
- Completes analysis of surveys as needed General office duties including copying, faxing, filing and word processing.
- Updates forms and other literature as assigned.
- Serve as backup for Meals on wheel delivery drivers.
- May provide support/services for other FNS programs such as MTM or FMOW.
- Upholds HIPAA office procedures and confidentiality.
- Performs other duties as assigned
EXPERIENCE AND TRAINING:
- High School Diploma or General Education Degree, Associates degree preferred or equivalent experience.
- Minimum one year of progressively responsible administrative/office environment.
- Experience in a community health field is preferred.
- Proficient in Microsoft Office products/software and Google applications, experience with Salesforce a plus.
- Bilingual English/Spanish required
MINIMUM KNOWLEDGE AND ABILITY:
- Ability to maintain productive working relationships with clients, program staff and community partners.
- Excellent telephone, communication and interpersonal skills.
- Ability to communicate effectively with diverse populations. Must be culturally sensitive.
- Aptitude for learning new database software.
- Ability to concentrate and pay close attention to detail.
- Ability to punctuate, spell, proof and edit own work.
- Ability to work independently, prioritize and handle multiple tasks.
- Ability to model and communicate The Health Trust’s mission and support a culture of empowerment and teamwork.
- Ability to show understanding, patience and tact when dealing with clients, referrers and community partners.
- Knowledge of standard office equipment, i.e., photocopier, fax.
WORKING CONDITIONS:
- Professional office environment.
- May perform repetitive motion activities and sitting for long periods of time.
- Must have a car available for daily use, a valid California driver’s license and current automobile insurance
