Monday - Friday, 8:30am - 5:00pm
Pay rate is $21.00 per hour
Provide oversight and excellent care and services to a case load of patients.
No experience required, will train willing individuals. Medical knowledge is a plus.

Duties include, but are note limited to:
- Take patient vitals, daily
- Collaborate with healthcare providers to ensure effective and efficient delivery of care
- Monitor patient progress and make necessary recommendations to Care Manager
- Provide education and support to patients and their families
- Utilize utilization management techniques to optimize healthcare resources
- Perform diagnostic evaluations and recommend appropriate treatment options
