POSITION SUMMARY:
The primary purpose of the Patient Surgery Navigator, is to help the patient navigate the DHR Health system to obtain all information necessary prior to their surgery, they are to be responsible for the following:
¨ Providing support and guidance through the patient's surgical needs leading up to surgery day
¨ Assist the patient with navigating through our network of physicians, clinics, hospitals, outpatient centers, and other components of our health care system
¨ Assist the medical team in educating the patient and family on resources available concerning treatment and diagnosis.
¨ Ensures that all needs of patients are addressed prior to surgery
¨ Assist patients with external resources referral appointments or other needs
POSITION EDUCATION/QUALIFICATIONS:
¨ High School graduate/GED certificate or equivalent is required
¨ Bachelor's Degree or course work toward a Bachelor's preferred
¨ Previous experience in a health care setting preferred
¨ Previous experience in working on interdisciplinary teams preferred
¨ Staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit
¨ Good organizational skills are necessary
¨ Good written and verbal communication skills required
¨ Ability to read, write and speak English
¨ Bi-lingual in Spanish
¨ Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians
¨ Professionalism, good demeanor and presentation required
JOB KNOWLEDGE/EXPERIENCE:
¨ Two (2) years experience working with patients in a hospital or MD office setting
¨ Able to communicate effectively in English and Spanish
¨ Good overall clerical skills, ability to work productively and effectively
¨ Ability to function well as a member of a multidisciplinary team
¨ Strong interpersonal skills
¨ Basic computer knowledge including Microsoft Excel and Word
POSITION RESPONSIBILITIES:
¨ Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices
¨ Initiates communication with patient upon receiving a referral of a patient for surgery
¨ Promote supportive relationships with the patient and family, to help navigate all needed documents prior to surgery
¨ Outlines needs and identifies resources to meet those needs for each patient
¨ Provides patients with education, and decision making support related to the impact of medical illness, social needs, functional needs, or financial needs on healing and recovery
¨ Initiates, facilitates and works with internal and external resources as to meet the identified patient needs
¨ Obtains orders from the physician for the individual patient and work with them on obtaining appointments needed to be "cleared" for surgery
¨ Continues to follow up with patient to ensure all pre surgery appoints are attended in order to obtain " medical clearance" for surgical procedure
¨ Prepare orders and all related diagnostic reports from pre-surgery medical work up for Financial Counselor &/or Surgery Scheduler
¨ Follows up with PCP to ensure that they are aware the their patient will be having surgery and what process and diagnostic procedures are needed prior to surgery
¨ Acts as a liaison between physician, hospital, OP diagnostic areas, the office and the patient and his family previous to surgery
¨ Other duties as assigned
